Tessera currently serves non-Medicare adults receiving care while physically located in Florida. This brief screening protects both the patient and the practice.
Required before every clinical appointment
Florida Location Attestation
Before Tessera provides diagnosis, treatment, or prescribing, every patient must electronically attest that they are physically located in Florida at the time of the appointment and are either:
- A Florida resident; or
- A visitor temporarily present in Florida.
“I attest that I am physically located within the State of Florida for this appointment. I am either a Florida resident or currently visiting Florida. I understand that Tessera cannot provide clinical treatment or prescribing while I am outside Florida, and that my reported location will be documented in my medical record.”
The patient's current Florida location will be reconfirmed at the beginning of every visit. If the patient is outside Florida or declines to complete the attestation, the clinical appointment will not proceed.

Completion of this eligibility form does not establish a physician-patient relationship, guarantee acceptance into the practice, or constitute medical advice.
