Local eligibility and house-call policy: The member must maintain an approved residential service address in Sarasota County or Manatee County. House calls are provided by appointment and are subject to clinical appropriateness, safety, location, weather, travel conditions, membership benefits, and physician availability. Concierge house calls are not emergency, 24/7 on-demand, or one-time services.

Founding membership is limited to the first 30 eligible Florida adults, with the selected membership rate locked while continuously active and in good standing. Monthly options require a 12-month commitment and applicable processing fees. Membership is not health insurance. Laboratory, imaging, prescription, specialist, hospital, and emergency-service charges are not included.
